How can I get a paper copy of the questionnaire?

If you would like to take part using a paper questionnaire, please contact our helpline team on Freephone 0800 819 9135 or email GPPatientSurvey@ipsos.com by 5pm on 26 January.

After this date, you can take part online at www.gp-patient.co.uk/survey (when the survey is open). If you have lost your survey code, please email GPPatientSurvey@ipsos.com for your survey code and we will email this back to you. This may take a few days as all survey codes are held securely.

You may also receive text messages, emails, WhatsApp or NHS App reminders inviting you to take part in the survey. These reminders will contain a unique link which enables you to take part online without needing to enter your survey code. Some text messages will include a link to an invitation e-letter, which you will be able to view online, explaining how to take part in the survey. This will also enable you to take part online without needing to enter your personal survey code.

Do I put a tick or cross in the box?

To help us capture your answers, please put a cross in the box or boxes for the answer that comes closest to your own opinion or experience.

What happens if I need help filling in the questionnaire on paper?

Ipsos provides a Freephone survey helpline for anyone who needs help filling in the questionnaire. Please call 0800 819 9135 between 8am and 9pm Monday to Friday, or 10am to 5pm on Saturdays.

Do I need to send the letter back?

No, you only need to send your filled in questionnaire back in the Freepost envelope provided.

When do I need to send it back by?

Please return your filled in questionnaire as soon as you can, but please make sure that it arrives with us before 23 March 2026.
