Do I put a tick or cross in the box?

You can put either a tick or a cross in the box or boxes for the answer that comes closest to your own opinion or experience.

What happens if I have problems filling in the questionnaire on paper?

Ipsos MORI provides a Freephone survey helpline for patients who have problems filling in the questionnaire. Please call 0800 819 9135 between 8am and 9pm Monday to Friday, or 10am to 5pm on Saturdays.

Patients can fill in the questionnaire online via the link on (when the survey is open). Please note however, that only one response per patient can be accepted.

You can also fill in the survey online in one of 14 different languages or British Sign Language. Also, paper versions are available in Braille and large print on request by calling the helpline number above or by emailing

I’ve lost my questionnaire. Can you send me another copy?

In February and March we will send reminder questionnaires to everyone who hasn't filled in the survey.

You can also take part online; please email us at for your survey number and password and we will email these back to you. This may take a few days as all survey numbers and passwords are held securely.You may also receive text message reminders inviting you to take part in the survey. Each text message contains a unique link, which includes your log-in details. This will allow you to take part online without needing to enter your survey number or password.

Do I need to send the letter back?

No, you only need to send your filled in questionnaire back in the Freepost envelope provided.

When do I need to send it back by?

Please return your filled in questionnaire as soon as you can, but please make sure that it arrives with us before:

I have not received an envelope/questionnaire with my letter.

We are sorry if this has happened. We will shortly be sending a reminder letter along with a copy of the questionnaire and another envelope. Please fill in the survey when it arrives and use the enclosed envelope to return it. If you have a questionnaire (but no envelope) and would like to send it to us before then, please put the questionnaire in an envelope marked only “FREEPOST GP PATIENT SURVEY”. You do not need a stamp.
