Online Questionnaire

Some people prefer to take part in the survey this way. If you have been invited to take part in the survey and would like to fill in the survey online, you do not need to return a paper questionnaire. Only one questionnaire per person can be accepted.

The online survey is available in English, 13 other languages and in British Sign Language. The survey is also fully compatible with W3C accessibility standards.

You can complete the questionnaire online by clicking the links below. You will then be prompted to enter your reference/ username and online password. These can be found on the first page of your paper questionnaire.
