Why haven’t I received a questionnaire? How do I get one?The questionnaire is being sent to a random selection of people who are registered with a GP in England. The Department of Health selected their names randomly from the NHS list of patients registered with a GP. To ensure the survey is valid, Ipsos MORI cannot send questionnaires to people who have not already been selected at random to take part in the survey.
I am not registered with a GP at the moment, should I still complete the questionnaire?
This survey is being sent to people who have been registered with a GP surgery in the last six months. So if you were registered with a surgery during that period, even if you are not now, you may still complete the questionnaire. Please answer the questions about the surgery you were registered with during that period (or most of that period).
I have not contacted my GP recently, should I still complete the questionnaire?
Yes, you can still complete the questionnaire. Although not all questions will apply to you we hope you will take part in the survey.
