Can I complete the survey online?If you would prefer to complete this survey online, please click here and follow the link to the survey. You will need to select your chosen language and enter your 11 digit reference number/ username when prompted for the username, as well as your unique password. The reference number and password are printed on the first page of the questionnaire. Please note however, that only one questionnaire per patient can be accepted.
I've lost my login details/reference needed to complete the survey online. Can I have these again?
In August and September we will send a reminder to everyone who hasn't filled in the survey. You will then receive another copy of the questionnaire and may choose to complete the survey on paper or use the reference to complete it online.
Where can I find the username/reference number for completing the survey online?
The username/ reference number and password for completing the survey online are printed on the first page of the questionnaire in the section marked with a computer symbol. Please click here for further guidance on where to find the username/ reference number and password.
