Can I complete the survey online?If you would prefer to complete this survey online, please click here and follow the link to the survey. You will need to select your chosen language and enter your 10 digit reference number when prompted, as well as your unique password. The reference number and password are printed on the first page of the questionnaire. Please note however, that only one questionnaire per patient can be accepted.
Where can I find the reference number for completing the survey online?

I've lost my login details/reference needed to complete the survey online. Can I have these again?
In February 2012 and again in March 2012 we will send reminder questionnaires to everyone who hasn't filled in the survey. You may choose to complete the survey on paper or use the reference to complete it online. Please remember to complete your survey by the closing date which is 30th March 2012.
